To Enroll in the O.H.H.H.I.T. Self-Pay Insurance Plan:
(A) Current Member of the OHHA

(B) Must reside in the State of Ohio.

(C) Licensed Trainer or Driver who earns 75% of their income driving and or training harness horses. With 50% of programed starts or at least 150 programed starts per year at Ohio racetracks and/ or County fairs. True stake races are exempt. Early closers and late closers are not exempt.

(D) If you have had any prior health insurance coverage, Legacy Management Group, LTD requires a Certificate of Health Coverage.

(E) If you are a late applicant (applying for coverage after 31 days of becoming eligible for coverage through the Ohio Harness Horsemen’s Health Insurance Trust, or after 31 days following the termination or exhaustion of prior Insurance) there will be a 6 month waiting period for insurance coverage.

The Health Insurance Portability and Accountability Act of 1996 (HIPAA) limits the circumstances under which coverage may be excluded for medical conditions present before you enroll. Under the law, a pre-existing condition exclusion generally may not be imposed for more than 12 months. The 12-month exclusion period is reduced by your prior health coverage. You are entitled to a certificate that will show evidence of your prior health coverage and can be obtained by contacting your previous plan administrator.

A copy of the certificate, a completed enrollment application and a check for the single or family coverage premium should be returned to the office at your earliest convenience.

Insurance coverage will become effective the first day of the month following a 30-day waiting period, being aware the final decision as to eligibility will be made only if all conditions (above) have been met. As stated in item (D) the waiting period will be 6 months for late applicants.

Premium rates:
Single Coverage - $415.00 per month

Family Coverage - $969.00 per month

(Your check will not be deposited until we receive notification from Legacy Management Group, LTD. that your insurance has been approved. If the check is returned by the bank for insufficient funds this will result in the cancellation of this insurance application).

This insurance coverage will become void at age 65 or Medicare eligible. The Ohio Harness Horsemen’s Health Insurance Trust does not carry a supplement insurance for Medicare.
